Program Supports Coordinator
PRIMARY FUNCTION:
To develop, coordinate, train, and monitor individual’s programs and perform quality assurance on the ICF Case Management functions.
RESPONSIBILITIES:
- To write Individuals Support Plan (ISP) and staff instructions in the ICF programs.
- To coordinate the development of data sheets that are needed to implement the ISP and other care plans.
- To communicate with members of IDT, evaluate data, and document the effectiveness of an individual’s outcome/action steps, adaptive equipment and therapies.
- To carry out staff ISP training for all individuals.
- To monitor the effectiveness of the ISP programs through communication with shift supervisors and on-site observations to ensure active treatment and coordinate changes as indicated.
- To monitor individual’s plans and record to ensure an integrated plan among all members of the Interdisciplinary Team (IDT).
- To monitor and follow-up on recommendations of ISP revisions, treatments, ordering of materials needed to carry out the ISP and recommendations of the various therapies.
- To ensure that recommendations are considered by the IDT (medication review, health services reports, nutrician, pharmacy review and therapy reports).
- To communicate with members of IDT, evaluate data, and document the effectiveness of an individual’s outcome/action steps, adaptive equipment and therapies.
- To write Monthly Reviews for all individuals ensure programs have been modified or changed in response to individual’s specific accomplishments, need for new program, pr difficulties in acquiring or maintaining skills.
- To monitor the ISP process to ensure it includes a comprehensive functional assessment (CFA), medical review, health services report, self-administration medication assessment, nutritional and therapy’s assessments completed in the required time frames.
- To monitor the ISP to ensure it reflects and responds to the functional changes which have occurred since the last ISP and the ISP is based on the CFA.
- To keep tracking logs of routine assessments for timely submission and completeness.
- To maintain documentation of contacts.
- To monitor mealtimes and provide retraining to staff and supervisors as indicated.
- To assist in state survey of the ICF programs and in developing the Plans of Correction for the survey.
- Complete on-site observation to ensure the Plans of Correction are being implemented.
QUALIFICATIONS
- Bachelor’s Degree in Education or Human Services Area
- At least one year of experience with individuals with Intellectual Disabilities
- Ability to be insured as a vehicle driver by agency’s insurance program
- A valid driver’s License
- Good knowledge of basic English sufficient to read instructions, keep records and write reports
- Successful completion of background and reference check
